To Make an Appointment
For your convenience, office hours are from 8:30am to 5:00pm, Monday through Friday and Saturday by appointment. If you find it necessary to change an appointment, please call us at least 24 hours in advance

New Patients
We ask you to arrive 15 minutes prior to your appointment to complete the necessary registration process. *If your insurance carrier requires a referral from your primary care physician, please be certain to bring it with you.* A prescription is required for Medicare patients. Bring copies of previous audiograms if available.		Insurance HTA is a participating provider with most major insurance carriers for diagnostic services.
